Requesting Your Health Care Records
You may request copies of your health care records by contacting this practice directly. Requests may be submitted via secure message, phone, or written request. Records requests are processed in accordance with Texas and federal law.
Behavioral Health Complaints (Texas Behavioral Health Council)
Complaints against a Texas behavioral health licensee must be submitted using the Texas Behavioral Health Council (BHEC) complaint form.

Consumer Complaints (Texas Attorney General)
You may file a consumer complaint with the Texas Office of the Attorney General’s Consumer Protection Division.
